Adherence Care Navigator 2

This is a fully remote position, open to applicants in Florida.

📋 Description

• Make 80–100 structured outbound calls each shift/day to Medicare members, pharmacies, and healthcare providers.

• Recognize and address medication adherence challenges for patients managing chronic conditions such as diabetes, hypertension, hyperlipidemia, and asthma.

• Adhere to structured call protocols.

• Deliver patient education within established guidelines.

• Collaborate with pharmacies and healthcare providers as necessary.

• Record all interactions in real time across various systems.

• Uphold quality and professionalism in a metrics-driven, high-volume call center setting.

⛳️ Requirements

• Current CPhT certification from PTCB and national certification.

• High School Diploma or GED required.

• At least 2 years of pharmacy or related patient interaction experience.

• 2–5 years of experience in pharmacy and extensive outbound calling.

• Proficient in Microsoft Word, Excel, Outlook, and Google applications.

• Capable of learning new technologies.

• Experience with Medicare/managed care is preferred.

• Ability to work independently in a remote setting.

• Proficiency in English and a second language for bilingual candidates; Spanish, Mandarin, Korean, Cantonese, or Vietnamese are advantageous.

• Willingness to work full time, 40 hours per week, Monday–Friday, during the shift range of 8:00 AM–8:00 PM CST.

• Must have the ability to connect directly to a router/modem; T-Mobile, satellite, and 5G internet connections are not permitted.

• Internet speed must be at least 50 Mbps download and 10 Mbps upload.

• A quiet, distraction-free workspace is required for remote work.
