This is a fully remote position, open to applicants in Florida, +3 more states.

📋 Description

• Perform thorough telehealth evaluations for senior patients with chronic and complex health issues.

• Detect clinical risks and identify care gaps across various diagnoses, medications, and recent healthcare utilization.

• Manage patient care plans collaboratively with healthcare advocates, addressing both medical requirements and social determinants of health.

• Work in conjunction with therapists and care teams to ensure that physical health is seamlessly integrated into ongoing treatment.

⛳️ Requirements

• MD or DO, licensed in a minimum of 45 states, including Florida, Maryland, Pennsylvania, and Texas.

• Board certification in Internal Medicine, Family Medicine, or a related specialty.

• Availability for at least 5 hours per week.

• Experience in managing patients with chronic and complex conditions, particularly among the elderly.

• Excellent clinical judgment with the capability to synthesize incomplete information into coherent plans.

• Proficient with technology and enthusiastic about utilizing AI to enhance care delivery.
