Wellness Associate II

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Facilitates population management initiatives for community or practice/facility-based settings.

• Designs and develops programs that ensure competencies, training prerequisites, reporting, and documentation.

• Provides support to regional teams and operational functions.

• Creates and implements tools for assessing the effectiveness of wellness programs.

• Offers on-site screening assistance and education as required.

• Represents the facility at various community events.

• Manages member communication and outreach programs.

• Conducts research and develops new program initiatives.

• Bachelor's Degree in a Healthcare Related Field (Required).

• A minimum of 1 year of relevant work experience (Required).

• Skills: Proficiency in Computer Literacy, Critical Thinking, and Interpersonal Communication.
