This is a fully remote position, open to applicants in Pennsylvania.
• Perform thorough 40-minute initial evaluations for patients seeking medical weight management.
• Conduct 20-minute follow-up appointments concentrating on medication management, titration, monitoring, and continuous clinical support.
• Assess patients for suitable GLP-1 therapy and prescribe medications within the scope of practice.
• Track treatment responses, side effects, laboratory results, and overall progress of patients.
• Collaborate with referring Primary Care Providers and Registered Dietitians to create personalized care plans.
• Inform patients about medication expectations, safety precautions, lifestyle modifications, and strategies for long-term weight management.
• Ensure timely and precise documentation within electronic health record systems.
• Adhere to Sylvan clinical standards and partner-specific workflows.
• Engage in regular team meetings and collaborate on program development and quality improvement initiatives.
• Contribute to the continuous advancement of Sylvan's medical weight management program and clinical protocols.
• Maintain a steady patient panel and appointment schedule.
• Provide high-quality, patient-centered care while fostering access and continuity across partner organizations.
• Current Nurse Practitioner certification (FNP, AGNP, or Adult-Gerontology Primary Care NP preferred).
• Active, unrestricted Nurse Practitioner license in relevant states or willingness to acquire additional state licensure.
• DEA registration or capability to obtain one.
• Experience in managing patients with obesity or metabolic disorders.
• Familiarity with GLP-1 receptor agonists and other anti-obesity medications.
• Background in medication titration, monitoring, and patient education.
• Strong clinical judgment and evidence-based decision-making capabilities.
• Exceptional communication and patient counseling skills.
• Comfortable working autonomously in a remote telehealth setting.
• Proficient in using electronic health records and telehealth platforms.
• Collaborative approach for working with referring Primary Care Providers and Registered Dietitians in an interdisciplinary care model.
• Flexibility and enthusiasm for helping to develop new clinical programs and workflows.
• Part-Time salaried W-2 position (20 hours weekly).
