This is a fully remote position, open to applicants in Arizona, +3 more states.
• Conduct detailed and targeted evaluations of patients with hypertension, hyperlipidemia, coronary artery disease, and diabetes who are nearing the end of their medication supply.
• Carry out virtual consultations, which are expected to constitute 40–60% of clinical activities.
• Develop diagnoses and personalized treatment strategies, including medication management, lifestyle changes, and monitoring suggestions.
• Oversee a prescription refill inbox and queue in accordance with established protocols and guidelines.
• Engage in centralized results management, which includes reviewing, triaging, and following up on laboratory and diagnostic results.
• Utilize digital live portal chat and AI-driven clinical tools for prescribing, inbox triage, medication renewal, results management, and patient communication.
• Recognize risk factors, comorbidities, and contraindications to treatment.
• Advise patients on medication adherence and possible side effects.
• Collaborate with primary care providers, specialists, pharmacy staff, clinical nursing, social workers, family members, and caregivers.
• Employ the company’s electronic health information system for virtual visits and provide feedback on the interface.
• Ensure accurate patient medical records are maintained in compliance with legal and ethical standards.
• Take part in quality improvement initiatives and continuous professional development.
• Comply with laws, regulations, industry standards, patient privacy requirements, and telehealth regulations.
• Advanced Practice Provider (NP or PA) credentials.
• At least three years of experience in outpatient clinical practice.
• Minimum of two years focused on primary care or a relevant subspecialty with a strong emphasis on hypertension and lipid management.
• For Nurse Practitioners: Must hold active and good-standing RN and APRN licenses.
• For Physician Assistants: Must have an active and good-standing Physician Assistant license.
• Licensure and certification must be in good standing.
• Capability to acquire licenses in the requested states within 90 days of hire.
• Experience in managing hypertension, hyperlipidemia, and both primary and secondary prevention of atherosclerotic cardiovascular disease.
• In-depth knowledge of cardiometabolic condition management, including disease processes, treatment options, medication management, and lifestyle modifications.
• Comfortable using digital live portal chat and AI-based clinical tools.
• Familiarity with STARS/HEDIS measures, coding clinical comorbidities, and identifying clinical care gaps.
• Preference for candidates with virtual care experience.
• Experience or strong interest in centralized results management, digital/chat-based patient engagement, and AI-enabled clinical workflows is advantageous.
• Proficiency in both English and Spanish is preferred.
• Availability to work three 12-hour shifts weekly, including weekends, from 8am–8:30pm EST.
