This is a fully remote position, open to applicants in Ohio.
• Become a part of a diverse obesity medicine team that delivers virtual care to Medicare and Medicaid patients.
• Perform thorough nutrition assessments through virtual and telehealth consultations.
• Offer Medical Nutrition Therapy (MNT) for obesity and associated chronic conditions.
• Create personalized nutrition care plans tailored to individual patient needs.
• Provide nutrition education and counseling utilizing motivational interviewing and behavior-change strategies.
• Enhance patient engagement through effective communication and appropriately escalate clinical concerns.
• Work collaboratively with physicians, nurse practitioners, health coaches, and care coordinators.
• Engage in care planning and case reviews as necessary.
• Record encounters, assessments, and care plans accurately in the EHR.
• Utilize telehealth and patient engagement tools efficiently.
• Play a role in the development of clinical programs, quality initiatives, and innovative interdisciplinary care as the organization expands.
• Current RD/RDN credential in good standing.
• Active LD/LDN or CD/CDN license in the relevant state(s) of practice.
• Bachelor’s or master’s degree in Nutrition/Dietetics or a related discipline.
• Required clinical/ambulatory experience in providing nutrition counseling or MNT.
• Preferred experience in obesity, weight management, chronic conditions, and telehealth.
• Excellent patient-centered counseling and documentation abilities.
• Ability to work effectively within interdisciplinary teams.
• Proficient with EHRs and telehealth platforms.
• Stable internet connection.
• A private workspace that complies with HIPAA regulations.
• Maintain licensure standards and adhere to HIPAA compliance.
