This is a fully remote position, open to applicants in Arizona, +3 more states.
• Provide compassionate clinical care through real-time and asynchronous chat, along with other responsive care channels.
• Handle multiple patient conversations simultaneously while ensuring clinical precision, timely documentation, and follow-up.
• Guide and supervise patients’ medical management using a comprehensive framework, including the 5M approach.
• Collaborate with primary care providers, specialists, pharmacy teams, and specialty care groups.
• Coordinate care with patients’ family members and caregivers when appropriate.
• Evaluate symptoms, address medication inquiries, interpret lab results, and manage new health concerns; offer clinical guidance and follow-up.
• Refer patients to telephone, video, in-person, urgent, or emergency care as necessary.
• Take ownership of patient needs until resolution or documented handoff.
• Ensure precise and timely EHR documentation in compliance with legal, ethical, and telehealth standards.
• Enhance patient experience and outcomes through digital workflows and proactive communication.
• Contribute to the development of standardized responses, escalation pathways, digital workflows, and quality benchmarks.
• Provide flexible clinical support across Responsive Care queues and on-call services.
• Occasionally assist broader Devoted Medical service lines.
• Comply with all applicable laws, regulations, privacy requirements, and telehealth standards.
• Advanced Practice Provider (NP or PA) with three or more years of experience in primary care, house calls, internal medicine, family medicine, geriatrics, palliative care, and/or urgent care.
• Capability to assess AI-generated responses for clinical accuracy and safety.
• Nurse Practitioners must possess active, in-good-standing RN and APRN licenses.
• Physician Assistants must maintain an active, in-good-standing Physician Assistant license.
• Must be able to acquire licenses in the requested states within 90 days of hire, starting from the signing of the offer letter.
• Must obtain additional state licenses as required.
• Willingness and ability to work 3x12s or 4x10s, including coverage on Saturdays and Sundays.
• Previous experience in virtual care or telehealth is preferred.
• Experience in high-volume telehealth clinical settings, including real-time and queue-based environments.
• Proven ability to work with interdisciplinary care teams and coordinate remote care.
• Comfort in managing complex or vulnerable patient populations in a digital-first environment.
• Flexibility and curiosity in a dynamic, rapidly evolving workplace.
• Must reside within one of the contiguous 50 states.
