Virtual Nurse Practitioner, Bilingual Spanish

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform initiating consultations via telehealth for E/M or Annual Wellness Visits (AWVs)

• Determine medical necessity for care navigation services

• Recognize high-risk conditions and unmet social needs

• Record findings in Baba’s platform

• Ensure precise coding and application of SDOH Z-codes, diagnoses, and risk factors

• Create and validate personalized care plans

• Verify that care plans are consistent with patients’ medical treatment strategies and document ongoing medical necessity

• Work together with care advocates and nurse care managers to finalize personalized care plans

• Enroll patients in a safe, compliant, and compassionate manner into the suitable level of support

⛳️ Requirements

• Completion of an accredited Nurse Practitioner program

• Active Nurse Practitioner or APRN license in good standing

• Licensed in multiple states

• Over 2 years of experience in primary care, telehealth, or care management

• Knowledge of Medicare programs, including care navigation, incident-to requirements, and SDOH documentation

• Proficient with EMRs, care management platforms, and telehealth workflows

• Bilingual in Spanish is required

• Legally authorized to work in the US

• 1099 contractor arrangement
