Virtual Nurse Practitioner – Bilingual Mandarin

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct brief and structured telehealth initiation visits.

• Perform telehealth-based E/M or Annual Wellness Visits to establish the medical necessity for care navigation services.

• Identify high-risk conditions and unmet social needs.

• Document findings using Baba’s platform.

• Ensure accurate coding and the application of SDOH Z-codes, diagnoses, and risk factors.

• Develop and approve personalized care plans.

• Ensure care plans are in alignment with patients’ medical treatment plans and document ongoing medical necessity.

• Collaborate with care advocates, nurse care managers, and the clinical operations team.

• Assist in enrolling patients safely, compliantly, and compassionately into suitable care navigation support.

⛳️ Requirements

• Completion of an accredited Nurse Practitioner program.

• Active Nurse Practitioner or APRN license in good standing, with licensure in multiple states.

• A minimum of 2 years of experience in primary care, telehealth, or care management.

• Familiarity with Medicare programs, including care navigation, incident-to requirements, and SDOH documentation.

• Proficient in using EMRs, care management platforms, and telehealth workflows.

• Bilingual in Mandarin is required.

• Legally authorized to work in the United States.

• No reportable license restrictions, disciplinary actions, loss of privileges, DEA actions, or malpractice payments to the NPDB.

• Availability for approximately 0–10, 11–20, or 21+ hours per week.
