This is a fully remote position, open to applicants in Wisconsin.
• Utilizes the nursing process alongside established protocols to evaluate the needs of patients via telephone.
• Identifies the most suitable level of care required, offers comprehensive education, establishes a care plan with interventions, and conveys follow-up instructions to the patient.
• Elevates and collaborates with the appropriate on-call provider when further guidance is necessary.
• Prioritizes patient interactions based on acuity and necessity, taking into account all available information and resources.
• Employs evidence-based practices to provide patient care.
• Implements strategies aimed at minimizing patient risk and enhancing safety.
• Evaluates patient and family readiness to learn, tailoring the approach as needed.
• Collaborates to create strategies that address the learning needs of both the patient and their family.
• Engages in shared governance activities and initiatives to enhance processes and patient outcomes.
• Participates in departmental quality and process improvement initiatives focused on enriching the patient care experience.
• Communicates relevant information regarding the patient's physical and psychological condition to the physician, Advanced Practice Clinician, and/or other members of the interdisciplinary team as appropriate.
• Delivers concise and relevant reports detailing the patient's response to medical and nursing care plans.
• Candidates MUST possess a minimum of 2 years of acute care experience (ED, Urgent Care, some ICU, and Med/Surg will be considered) within the last 4 years to qualify.
• Must hold a Registered Nurse license issued by the state in which the team member practices.
• An Associate's Degree (or equivalent knowledge) in Nursing is required.
• Strong critical thinking skills are essential to independently assess and prioritize patient needs using sound judgment and effective problem-solving abilities.
• Familiarity with various healthcare specialties, including levels of care, symptom recognition, and validated treatment recommendations.
• Capability to integrate past experiences with established protocols.
• Exceptional verbal communication skills that reflect empathy, respect, restatement, open-ended questioning, active listening, and diplomacy with a diverse customer demographic.
• Ability to build rapport and maintain positive, professional relationships with a wide range of patients, staff, and physicians.
• Proven skill in independently organizing and prioritizing tasks, managing multiple priorities, and adapting to a flexible schedule in a fast-paced, dynamic customer service environment.
• Outstanding customer service and follow-up abilities, including the capacity to remain composed in stressful situations.
• Demonstrated proficiency in technology, including computers, internet usage, desktop software packages, and multi-line telephone systems.
• Competence in interacting with customers/patients while documenting and researching calls across multiple systems.
• Understanding of documentation techniques for effective communication.
