
Triage Registered Nurse (RN) – Patient Access and Care Team

This is a fully remote position, open to applicants in United States.
• Utilize the nursing process and established protocols to assess patient needs over the phone.
• Identify the suitable level of care, offer education, create care plans and interventions, and relay follow-up instructions.
• Elevate cases and collaborate with on-call providers when further guidance is required.
• Prioritize patient interactions based on acuity and necessity.
• Implement evidence-based practices and strategies that minimize risk and enhance patient safety.
• Evaluate the readiness of patients and families to learn and tailor educational approaches accordingly.
• Support shared governance, quality improvement, process enhancement, and patient outcome initiatives.
• Schedule appointments in accordance with protocol recommendations.
• Coordinate medical and nursing management, procedures, and medication refills with healthcare team members.
• Maintain and update clinical records and document all call interactions in the Electronic Medical Record.
• Communicate patient status and concise care-plan reports to physicians, Advanced Practice Clinicians, and interdisciplinary team members.
• Engage in team meetings and special projects as directed by leadership.
• Contribute to health information resources, system efficiencies, and operational improvements.
• Assist in interpreting department policies and procedures and provide guidance to staff regarding procedural changes.
• Valid Registered Nurse license issued by the state in which the team member practices.
• Associate's Degree (or equivalent knowledge) in Nursing.
• At least 2 years of acute care experience, preferably within the last 4 years; experience in ED, Urgent Care, ICU, and some Med/Surg is considered.
• Strong critical thinking and problem-solving skills.
• Familiarity with healthcare specialties, levels of care, symptom identification, and treatment recommendations.
• Capability to apply established protocols and previous clinical experience.
• Excellent verbal communication skills, empathy, active listening, diplomacy, and customer service abilities.
• Ability to organize and prioritize multiple tasks in a fast-paced environment.
• Proficiency with computers, internet, desktop software, multiple-line telephone systems, and various systems for research and documentation.
• Knowledge of documentation techniques.
• Stable and secure internet connection.
• Functional vision, touch, speech, and hearing.
• Ability to sit for most of the workday.
• A quiet space for making and receiving phone calls.
• Ability to lift up to 15 lbs.
• Availability to work every 4th weekend and holiday rotation.
• Must be able to work an 8-hour or 10-hour shift, with start times ranging from 8–10 a.m. or 12–2 p.m. CST or later.
