This is a fully remote position, open to applicants in New Mexico.
• Offer telephone and electronic triage support to individuals and staff in need.
• Assist with the coordination of outpatient care.
• Handle incoming clinical inquiries from staff and patients via phone; appropriately triage and escalate emergencies without delay.
• Collaborate closely with peer mental health workers, social workers, and Advanced Practice Providers (APPs) to address acute issues in a comprehensive manner.
• Coordinate care with patients' other healthcare providers to ensure smooth health management.
• Create and conduct training sessions on fundamental medical topics for peer mental health workers and social workers.
• Current RN license through a Nurse Licensure Compact (NLC) state with a commitment to obtain licensure in non-compact states.
• Bachelor of Science in Nursing (BSN) or Associate of Science in Nursing (ASN).
• Minimum of 3 years of experience in an Emergency Room (ER) setting.
• Experience working with populations facing challenges such as severe mental illness, behavioral health conditions, and/or substance use disorders.
• Experience in care management and coordination.
• Physical and mental health support.
