This is a fully remote position, open to applicants in Virginia.
• Travels within the designated territory to perform in-home assessments, which encompass comprehensive annual wellness exams, targeted screenings, and other visits in both patients' homes and virtual settings.
• Willingness to travel out of state for periods of up to two weeks as necessary.
• Provides counsel and education to patients and their families regarding available benefits and programs designed to promote healthier living.
• Records essential information such as the chief complaint, relevant diagnoses, past medical history, family and social history, review of systems, examinations, medications, allergies, assessments, and care plans.
• Oversees the coordination of care with primary care providers, specialists, and relevant ancillary services.
• Ensures all documentation and paperwork are completed promptly.
• Upholds quality of care standards as established by the practice.
• This role is not office-based; it will be remote within the state of employment, requiring attendance at periodic training and meetings outside that state.
• Provides evidence-based, timely care aimed at minimizing avoidable hospitalizations, maximizing quality of life, and prioritizing patient health and satisfaction.
• Prescribes medications, orders tests, and collaborates with the patient's Monogram physician.
• Performs effectively as demonstrated by improved patient quality outcomes, which will be measured and reported on a daily basis.
• Aids in closing gaps in care by educating patients on preventive monitoring and coordinating with physician practices to arrange diagnostic testing.
• Assists patients in enrolling to access educational videos.
• Participates in integrated care team meetings.
• Possesses knowledge of disease diagnosis and prevention.
• Assesses patients' health status.
• Develops treatment plans.
• Implements plans that align with the appropriate care strategy.
• Follows up and evaluates the patient's status.
• Other duties as assigned.
• Active and unrestricted Registered Nurse and Nurse Practitioner Board certification corresponding to appropriate licensure (NP: ANCC/AANP).
• Current and unrestricted DEA certificate.
• Willingness to travel up to 75% of the time.
• Ability to work independently and practice autonomously.
• Access to reliable transportation, a valid driver's license, and car insurance.
• Proficient in using medical instruments and equipment pertinent to the role.
• Familiarity with computer-based data management systems, information systems, and medical records as well as point-of-interview technology.
• Effective communication skills in both verbal and written forms with retail and medical partners, patients, family members, physicians, and community representatives.
• Solid understanding of all federal and state regulations, including HIPAA and OSHA.
• Minimum of two years of direct patient care experience required.
• Bilingual in Spanish is a plus, but not mandatory.
• Experience in Managed Care/IPA/Health Plan settings.
• Experience in conducting wellness visits, focused assessments, or similar comprehensive visits either virtually or in-home.
