
Transitions of Care Medical Assistant β Member Outreach

This is a fully remote position, open to applicants in United States.
β’ Engaging with patients through inbound and outbound calls post-discharge to assess their well-being and confirm they have received the necessary support.
β’ Collaborating with affiliated vendors to arrange services and care for members, including transportation, durable medical equipment (DME), meals, home health care, and more as needed.
β’ Aiding in the scheduling of follow-up visits for post-discharge Transition of Care (TOC) Advanced Practice Provider (APP), Primary Care Provider (PCP), and Specialist appointments.
β’ Working alongside patients, healthcare providers, and caregivers to promote positive health outcomes.
β’ Partnering with fellow members of the TOC team to meet patient needs and achieve team-based objectives, which may involve interdisciplinary huddles.
β’ Participating in and supporting pilot programs.
β’ Coordinating with healthcare providers and facilities to obtain essential discharge documentation.
β’ Adhering to escalation protocols to the clinical team to ensure patients receive necessary evaluations and care.
β’ A current and valid Medical Assistant Certification.
β’ 2-3 years of experience in healthcare, managed care, home health, and/or community health environments.
