This is a fully remote position, open to applicants in United States.
• Oversee the admission, transfer, and consultation processes for emergent, urgent, and elective patients.
• Assess patient acuity and utilize clinical decision-making to enhance patient flow.
• Record and prioritize patient details for accepting physicians, nurses, and healthcare personnel.
• Stay updated on departmental procedures, protocols, and communications.
• Determine transfer priority levels for patient triage and transportation.
• Manage Transfer Center data and offer feedback to referring facilities.
• Evaluate admissions for the appropriate level of care, patient status, and bed type.
• Implement Medical Necessity criteria.
• Provide education to nurses, physicians, and ancillary staff regarding level-of-care and admission/transfer matters.
• Act as a liaison among physicians, patients and families, hospital staff, patient-care areas, and external agencies.
• Ensure adherence to regulatory standards, EMTALA, ethics and compliance policies, and quality initiatives.
• Monitor and analyze barriers to patient flow; suggest process enhancements and formulate action plans.
• Raise concerns to Transfer Center leadership and facility administrative leadership.
• Collaborate with the Bed/Capacity Coordinator for timely transfers, bed assignments, and accurate patient data.
• Keep precise information in the Electronic Health Care Record.
• Engage in employee orientation, ongoing training, staff meetings, and continuous quality improvement projects.
• Manage inbound and outbound calls to achieve departmental productivity targets.
• Deliver exceptional customer service and support Transfer Center key performance indicators and Access Point Vision objectives.
• Graduate from an accredited school/college.
• Must possess current RN licensure in the state of residence.
• Required to maintain current nursing licensure and fulfill renewal and continuing education obligations.
• A minimum of three years of relevant clinical experience in an acute-care setting; experience in ED or Critical Care is preferred.
• Proficient in effective communication in oral, written, and electronic formats.
• Capable of multitasking while attentively listening and typing at a speed of over 35 words per minute.
• Must satisfy internet speed requirements: upload 50 or more; download 100 or more.
• Knowledgeable in human growth and development, assessment, treatment, and care for patients of all ages.
• Competent in applying Medical Necessity criteria.
• Familiarity with regulatory requirements, EMTALA, ethics and compliance policies, and quality initiatives.
• Experience with electronic health-care records and electronic resources.
• A Compact License is preferred.
• Must meet current nursing licensure standards; expired licenses will prevent scheduling.
• An Associate’s or bachelor’s degree is preferred.
