Therapy Continuity Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• This position is a non-clinical, operational role that collaborates closely with Sleep Technologists during the transition from treatment setup to long-term resupply.

• Once patients progress to the resupply phase, you will manage follow-up, execution, and documentation to ensure a seamless and dependable experience.

• Operate within a tech-enabled, AI-supported care environment.

• Work in conjunction with clinical, billing, and operations teams.

• Assist in tracking engagement and reorder metrics while contributing to a scalable operational foundation within a rapidly expanding digital sleep clinic.

• Implement timely and proactive outreach for resupply.

• Adhere to structured retention workflows.

• Ensure accurate tracking of reorders.

• Communicate clearly and compassionately with patients.

• Appropriately escalate any clinical or billing issues.

• Support efforts related to adherence monitoring.

⛳️ Requirements

• Minimum of 2 years experience in healthcare coordination, DME, or care navigation.

• Experience with PAP/CPAP resupply is a strong advantage.

• Excellent patient communication skills with a high degree of empathy and professionalism.

• Ability to work independently in a fast-paced, remote setting while effectively collaborating with cross-functional teams.

• Confidence in navigating digital tools and care platforms.

• Data-driven mindset with the capability to track tasks, maintain accurate records, and adhere to structured workflows.

• Comfort in managing proactive outreach and patient follow-up while maintaining a strong attention to detail.

• Genuine commitment to enhancing patient experience, therapy adherence, and continuity of care.

• Operational and automation-oriented mindset.

• Availability to work during Central Time (CST) business hours.
