Telehealth Preventative Primary Care Advanced Practice Provider – NP/PA

This is a fully remote position, open to applicants in United States.

📋 Description

• Primarily conduct telehealth video consultations, with occasional in-home visits to members in your vicinity.

• Execute thorough assessments and deliver evidence-based care aimed at prevention, chronic condition management, medication optimization, and health maintenance.

• Recognize and address care gaps, collaborate with both external and internal care team members, and contribute to personalized, holistic care plans.

• Leverage integrated technology and AI-driven workflows to minimize administrative workload and enhance meaningful interactions with members.

• Communicate with compassion and clarity, building trust and confidence with members and caregivers.

⛳️ Requirements

• Valid Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license.

• At least 3 years of outpatient clinical experience, preferably in primary care, family medicine, internal medicine, or geriatrics.

• Active and clear NP/PA license (must be prepared to obtain additional licenses if necessary within the first 90 days — support and reimbursement will be provided).

• Current BLS certification at the time of hire.

• Comfort with providing care through video telehealth and utilizing electronic documentation systems.

• Familiarity with managed care models, including STARS/HEDIS and the identification of care gaps (preferred).

• Experience in conducting comprehensive or preventive care visits with Medicare populations (preferred).

• Experience assisting older adults and/or individuals with complex medical or social requirements (preferred).
