Telehealth Preventative Primary Care Advanced Practice Provider – NP/PA

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Conduct predominantly telehealth video consultations, with occasional in-home visits to members within the local area.

• Carry out thorough assessments and deliver evidence-based care that emphasizes prevention, support for chronic conditions, medication optimization, and maintenance of health.

• Identify and resolve care gaps.

• Collaborate with both external and internal members of the care team.

• Contribute to personalized, whole-person care strategies.

• Leverage integrated technology and AI-enabled workflows to alleviate administrative burdens and enhance meaningful interactions with members.

• Communicate with empathy and clarity, building trust and confidence with members and caregivers.

• Work in partnership with primary care providers, specialists, pharmacists, nurses, care coordinators, caregivers, and families to provide coordinated, member-centered care.

• Foster relationships with members in a supportive, technology-enhanced environment.

⛳️ Requirements

• Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license.

• A minimum of 3 years of outpatient clinical experience, preferably in primary care, family medicine, internal medicine, or geriatrics.

• Valid and unencumbered NP/PA license.

• Must be prepared to obtain additional licenses if required within the first 90 days.

• Current BLS certification at the time of hire.

• Comfortable providing care through video telehealth platforms.

• Proficient in utilizing electronic documentation systems.

• Preferred: Knowledge of managed care models, including STARS/HEDIS, and experience in identifying care gaps.

• Preferred: Background in conducting comprehensive or preventive care visits for Medicare populations.

• Preferred: Experience in supporting older adults and/or individuals with complex medical or social needs.
