Telehealth Preventative Primary Care Advanced Practice Provider – NP/PA

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Primarily conduct telehealth video consultations, with occasional in-home visits to members within your local area.

• Carry out thorough assessments and deliver evidence-based care that emphasizes prevention, chronic condition management, medication optimization, and health maintenance.

• Identify and resolve care gaps, collaborate with both external and internal care team members, and contribute to personalized, holistic care plans.

• Leverage integrated technology and AI-driven workflows to lessen administrative tasks and enhance meaningful interactions with members.

• Communicate with empathy and clarity to build trust and confidence among members and their caregivers.

⛳️ Requirements

• Current Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license.

• Over 3 years of clinical experience in an outpatient setting, preferably in primary care, family medicine, internal medicine, or geriatrics.

• Valid and clear NP/PA license (must be prepared to obtain additional licenses if needed within the first 90 days — support and reimbursement will be provided).

• Active BLS certification required at the time of hiring.

• Comfortable providing care through video telehealth and utilizing electronic documentation systems.

• Familiarity with managed care models, including STARS/HEDIS and care gap identification (preferred).

• Experience in conducting comprehensive or preventive care visits with Medicare populations (preferred).

• Background in supporting older adults and/or individuals with complex medical or social needs (preferred).
