📋 Description

• Acquire knowledge of the Solace systems, tools, technologies, partners, and expectations, while contributing your unique clinical expertise in every interaction.

• Recognize and prioritize patients' needs to ensure a seamless care continuum.

• Manage patient care plans in collaboration with healthcare advocates to comprehensively address social determinants of health, such as food resources, transportation access, and home support.

⛳️ Requirements

• Board certification in family medicine, internal medicine, or another relevant specialty.

• Strong clinical abilities combined with exceptional organizational skills. You can juggle multiple tasks and thrive under pressure while maintaining clarity in your communications and documentation.

• Excellent communication skills, with a focus on listening and empathy as the foundation for every patient interaction at Solace.

• Equally adept at working with elderly patients and individuals facing Social Determinants of Health.

• A strong sense of technical proficiency. You can quickly and proficiently learn new systems and software.

• A proactive approach to action and execution. You are committed to creating a system that fosters improved patient outcomes and are unreserved in providing your feedback.
