Telehealth Physician

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Conduct routine telehealth consultations with Medicare patients (E/M and Chronic Care Management)

• Develop and oversee individualized care plans for ongoing health conditions

• Collaborate with our integrated care team, which includes nurse case managers, nurse practitioners, and community health workers

• Maintain comprehensive documentation in our electronic health records (EHR) and ensure adherence to quality standards

• Involve patients in preventive healthcare to minimize avoidable complications

⛳️ Requirements

• MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics

• Missouri License - residence is not a requirement

• Minimum of 1 year of experience as a physician in family medicine or internal medicine

• Active medical license — multi-state licensure is advantageous

• Experience in preventive care, senior health, or management of chronic conditions is preferred

• Proficient in practicing via telehealth and utilizing EHR systems is an advantage

• Prior experience in delivering preventive virtual care is a plus
