Telehealth Nurse Practitioner

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Perform thorough health assessments using telehealth technologies.

• Ensure proper documentation of risk adjustment (HCC coding) during patient interactions.

• Address HEDIS care gaps during patient appointments.

• Evaluate medical history, current medications, and preventive healthcare needs.

• Record visits utilizing ICD-10 and CPT II coding systems.

• Provide comprehensive care plans and follow-up instructions.

• Keep precise documentation of visits for quality assurance programs.

⛳️ Requirements

• Valid Nurse Practitioner license in Wisconsin.

• Preference for candidates with multi-state NP licenses.

• Minimum of 1 year of experience in Family Medicine or Internal Medicine.

• Experience working with Medicare and Medicaid patient populations.

• Knowledge of HEDIS standards and risk adjustment processes.

• Required enrollment as a provider for Medicare and Medicaid.

• Must be adept at providing care via telehealth platforms.

• Ability to consistently work during weekday daytime hours, with a preference for availability on Thursdays and Fridays.
