📋 Description

• Perform thorough health assessments via telehealth.

• Record HCC risk adjustments during patient appointments.

• Address HEDIS care gaps to enhance quality measures.

• Assess medical history, medications, and preventive requirements.

• Log visits utilizing ICD-10 and CPT II coding.

• Provide well-defined care plans and follow-up instructions.

• Ensure precise documentation for quality improvement programs.

⛳️ Requirements

• Valid Nurse Practitioner license in West Virginia.

• Over 1 year of experience in Family Medicine or Internal Medicine.

• Familiarity with Medicare and Medicaid patient populations.

• Knowledge of HEDIS and risk adjustment processes.

• Required enrollment as a provider with Medicare and Medicaid.

• Proficient in delivering care via telehealth platforms.
