Supportive Care Case Manager, Palliative Care RN

This is a fully remote position, open to applicants in Arizona, +4 more states.

📋 Description

• The DaVita Integrated Kidney Care Supportive Care Case Manager/Palliative Care (RN) acts as a clinical subject matter expert and provides specialized support for patients dealing with advanced Chronic Kidney Disease (CKD) and End Stage Kidney Disease (ESKD).

• Utilizes extensive knowledge in Palliative care to facilitate intricate Goals of Care (GOC) and advance care planning discussions, guiding patients towards comfort resources.

• Operates independently to assist the primary Integrated Kidney Care team with high-acuity patients, coordinating efforts between the dialysis center, Nephrologist, primary care, and external agencies.

• Conduct complex conversations and engage in thorough, empathetic telephonic discussions to inform patients about treatment options and ensure their preferences are respected.

• Evaluate symptom burden (pain, anxiety, dyspnea) and collaborate with providers to execute nursing interventions that enhance Quality of Life (QOL).

⛳️ Requirements

• A minimum of 2+ years of experience as an RN in Palliative medicine is required.

• At least five (5) years of experience as an RN is necessary.

• Must reside in Mountain or Pacific Time Zones exclusively.

• 2+ years of experience in care plan creation, proactive discharge planning, and utilization.

• A current RN License is required, with a BSN preferred.

• Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification is mandatory.

• Proficiency in EMR systems (Cerner), Microsoft Office (Teams, Excel), and virtual communication platforms is essential.

• A home office with internet connectivity of at least 1MB upload and 1MB download speed is required.
