This is a fully remote position, open to applicants in Washington.
• Provides leadership in both clinical and administrative capacities to care management teams.
• Oversees daily operations of medical and behavioral health clinicians, along with non-clinical staff, as they deliver care management services.
• Implements care management initiatives and ensures that program operations and documentation adhere to regulatory standards.
• Prioritizes daily workflows, assignments, and triage tasks to comply with state and federal regulations.
• Monitors team performance and productivity to guarantee adherence to guidelines and achievement of goals.
• Facilitates collaboration with internal partners for a unified approach to care coordination for members.
• Supports a regionalized care team structure to enhance care coordination across Integrated Managed Care (IMC) regions.
• Maintains a thorough understanding of health plan benefits.
• Participates in Care Management Rounds to foster productive discussions and provide clinical insights.
• A minimum of three (3) years of clinical experience in an acute care or public health environment.
• At least three (3) years of experience as a case manager within an Accountable Care Organization (ACO), Managed Care Organization (MCO), or Behavioral Health Organization (BHO) is required.
• Preferred: Three (3) years of supervisory or leadership experience within a healthcare or managed care context.
• A bachelor's degree in nursing or a master’s degree in social work and/or a related behavioral health field.
• A current, unrestricted RN license or a current, unrestricted LICSW license in the State of Washington.
• Must have or will obtain a Case Management Certification within the first two (2) years of employment.
• Proficiency in Microsoft Office applications, including Outlook, Word, and Excel.
