• Involve members in the care coordination and management process.
• Educate individuals about all available services, including natural and community supports, treatment alternatives, diagnoses, and more.
• Connect members to essential psychological, behavioral, educational, physical, and dental assessments.
• Facilitate referrals for health-related services, community resources, social support services, and community reintegration assistance.
• Assess the effectiveness of the care plan and reintegration services.
• Must fulfill Qualified Professional (QP) criteria.
• Fully licensed by the NC governing board that oversees Human Services professions; OR
• Fully licensed RN authorized to practice in NC by the NC Board of Nursing, possessing four (4) years of full-time experience with the MH/DD/SA population served.
• A graduate from an accredited college or university with a master’s degree in a human service field, along with one year of full-time, pre or post-graduate MH/DD/SA experience with the MH/DD/SA population; or a substance abuse professional with one year of full-time, pre or post-graduate supervised experience in alcoholism and drug abuse counseling; OR
• A graduate from an accredited college or university with a bachelor's degree in a human service field, having two years of full-time, pre or post-bachelor's degree MH/DD/SA experience with the MH/DD/SA population; or a substance abuse professional with two years of full-time, pre or post-bachelor's degree supervised experience in alcoholism and drug abuse counseling; OR
• A graduate from an accredited college or university with a bachelor's degree in a non-human services field, possessing four years of full-time, pre or post-bachelor's degree MH/DD/SA experience with the MH/DD/SA population; or a substance abuse professional with four years of full-time, pre or post-bachelor's degree supervised experience in alcoholism and drug abuse counseling.
• Additionally, if working with members requiring Long Term Services and Support (LTSS), the care manager must meet the minimum QP requirements outlined above (A-D) and have at least two (2) years of prior experience in LTSS and/or Home and Community Based Service (HCBS) coordination, care delivery monitoring, and care management. This experience may overlap with the duration required to become a Qualified Professional.
• The following immunizations are mandatory for this role: full vaccination against Covid-19, annual Influenza vaccination during the season (Nov 1 – March 31), MMR, Varicella, and Tdap (Tetanus, diphtheria, pertussis). Proof of MMR and Varicella vaccinations or blood tests showing immunity is required. Proof of one Tdap dose is necessary. Employees must provide documentation of all immunizations before their start date or agree to submit proof within four months of their start date.
• Must possess a valid driver's license.
• Must live within 75 miles of Wayne County, North Carolina.
• Must be willing to travel within the catchment area as needed.
• Standard working hours are 8:30 am – 5:00 pm; flexible schedules are available for some roles with management approval.
