This is a fully remote position, open to applicants in South Dakota.

• Works in collaboration with the Regional Director of Nursing and the Pharmacy Team to enhance and supervise nursing care coordination.

• Assess Intakes, prepare referrals, and arrange nursing care for patients with skilled RNs in either the Home or Infusion Center environments.

• Effectively communicate with patients and nursing staff to ensure the delivery of safe and high-quality services.

• Organize, maintain, and relay patient care information/documents to both internal and external personnel.

• Guarantee precise and clear documentation within EMR systems.

• Ensure adherence to external standards and regulatory requirements.

• Act as a liaison among interdisciplinary teams.

• Establish and assist in managing RN work schedules to meet staffing needs.

• Keep daily staffing reports and provide weekly updates to management.

• Carry out additional responsibilities as assigned.

• High school diploma or equivalent qualification.

• Experience in a staffing/scheduling coordinator role or a comparable position.

• Basic understanding of medical terminology.
