Social Work Care Manager

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Facilitate services for members who fulfill specific criteria.

• Focus on education/self-management while enhancing quality care and achieving cost-effective results.

• Evaluate member needs and explore available options for services and resources.

• Create and execute a care plan, coordinate resources, and track progress.

• Assess member status, manage discharges, and document the care management process and outcomes.

• Address medical, psychosocial, clinical, and behavioral health needs.

• Offer counseling and connect members to community/local/state programs.

• Work collaboratively with caregivers, providers, and community organizations to ensure coordinated care.

⛳️ Requirements

• A minimum of 3 years of clinical experience in social work counseling.

• Experience in managing complex psychiatric and substance abuse treatment or substance use disorders.

• Graduate from an accredited School of Social Work with a Master's degree in Social Work (MSW).

• Possession of a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) credential.

• Certification as an Independent Practice Social Worker (CIPSW) or Licensed Independent Social Worker (LISW).

• Licensed Professional Counselor (LPC) credential required.

• Must obtain Certified Case Management certification within 2 years of employment.
