This is a fully remote position, open to applicants in Canada.

• Evaluate referral requests to confirm compliance with established criteria

• Ensure all necessary data is gathered and recorded

• Participate in meetings with referral sources as needed

• Document and escalate any service-related issues

• Oversee the work of assigned clinicians to ensure adherence to standards and deadlines

• Assist the clinician network in managing expectations effectively

• Identify efficiencies and propose solutions to challenges

• Serve as a liaison between clients/patients, insurers, employers, and healthcare professionals

• Function as the initial point of contact for service providers and corporate clients

• Aid in managing and tracking complex cases

• Accurately and confidentially maintain databases and client/case files

• Bachelor's degree in a relevant field or equivalent experience and training

• Experience in Case Coordination and/or Case Management is highly advantageous

• Knowledge of Disability Treatment and Management principles, practices, and processes

• Proficient in Microsoft Word, Excel, and PowerPoint

• Exceptional verbal and written English communication abilities

• Effective self-directed work style with the capacity to operate with minimal supervision

• Strong administrative and customer service capabilities

• Excellent interpersonal skills with the ability to engage with diverse stakeholders

• Competence in managing multiple demands while adhering to time constraints and deadlines

• Flexibility to adjust to shifting priorities

• Ability to positively contribute to a collaborative work environment
