Service Coordinator

This is a fully remote position, open to applicants in Canada.

📋 Description

• Evaluate referral requests to ensure compliance with established criteria.

• Ensure that data is gathered and entered as required.

• Participate in meetings with referral sources as needed.

• Document and escalate any service-related issues.

• Oversee the work of assigned clinicians to ensure adherence to standards and timelines.

• Assist the clinician network in managing expectations.

• Identify efficiencies and propose solutions to challenges.

• Serve as a liaison with clients/patients, insurers, employers, and healthcare professionals.

• Act as the initial point of contact for service providers and corporate clients.

• Aid in the management and tracking of complex cases.

• Accurately and confidentially maintain databases and client/case files.

⛳️ Requirements

• A Bachelor’s degree in a relevant field or equivalent experience and training.

• Experience in Case Coordination and/or Case Management is a significant advantage.

• Proficient understanding of Disability Treatment and Management principles, practices, and processes.

• Strong skills in Microsoft Word, Excel, and PowerPoint.

• Exceptional verbal and written communication skills in English.

• A self-directed work style with the ability to operate with minimal supervision.

• Robust administrative and customer service abilities.

• Effective interpersonal skills with the capacity to communicate with various stakeholders.

• Capability to manage multiple demands efficiently under time constraints and deadlines.

• Flexibility in adapting to changing priorities.

• Ability to positively contribute to a collaborative work environment.
