This is a fully remote position, open to applicants in United States.
• Define and promote the roadmap for care transition features — encompassing hospital discharge planning through 30/60/90-day post-acute follow-ups, with an emphasis on skilled nursing transitions.
• Identify areas for improvement by integrating clinical evidence, patient journey research, and market intelligence, while discerning when feature requests indicate deeper architectural needs.
• Collaborate with product leadership to formulate the annual product strategy, OKRs, and investment priorities for the post-acute sector.
• Conduct ongoing discovery with clinical end-users — shadow care managers and nurses in their actual workflows, rather than solely conducting interviews.
• Design and execute swift experiments (prototypes, pilots, A/B tests) to confirm problem-solution fit prior to full-scale development, leveraging modern AI-assisted prototyping for expedited processes.
• Convert qualitative insights and quantitative data into well-defined, high-confidence hypotheses and actionable specifications for engineering teams.
• Oversee the entire sprint lifecycle — from story creation and acceptance criteria to UAT sign-off and release, utilizing our AI-centric toolsets to focus more on judgment and less on procedural tasks.
• Collaborate with engineering and design teams to deliver features that are clinically accurate, HIPAA compliant, and user-friendly.
• Maintain a balanced and prioritized backlog that addresses strategic initiatives along with technical debt and partner commitments.
• Define and monitor product KPIs linked to clinical outcomes, efficiency improvements, and patient experience.
• Establish feedback loops with customer success and clinical operations to ensure continuous improvement after launch.
• Report on product performance and key insights to executive, customer, and clinical leadership.
• 5+ years of experience in product management or product ownership, with at least 3 years in healthcare technology.
• Extensive domain knowledge in post-acute care, care transitions, or related fields (home health, SNF, hospice, chronic care management).
• Strong fluency and enthusiasm for AI-assisted and agentic product workflows, along with an interest in shaping the future of senior PM roles as tools evolve.
• Proven experience in delivering complex, regulated software products within Agile/Scrum environments.
• Demonstrated capability to engage with clinical stakeholders and translate clinical workflows into product specifications.
• Excellent analytical skills — proficient in SQL, product analytics tools, and outcome metrics.
• Familiarity with HIPAA, CMS regulatory frameworks, and interoperability standards (HL7 FHIR, ADT feeds) is preferred.
