• Provide leadership, guidance, and supervision for a multidisciplinary team delivering NC Medicaid Tailored Care Management (TCM) services to members involved in the justice system, addressing their behavioral health (BH), intellectual/developmental disabilities (I/DD), and/or substance use (SU) needs.
• Ensure the provision of holistic, integrated care management that aligns with NC Medicaid Tailored Plan requirements, encompassing comprehensive assessments, individualized care planning, care coordination, transitions of care, and health promotion.
• Train Coordinators and Health Specialists on the systems and processes essential for facilitating successful community re-entry.
• Leverage community resources to assist in connecting members and families to services that cater to their needs within their homes and communities.
• Oversee and support staff responsible for coordinating and managing care for participants in the Transitions to Community Living program.
• Lead and assist in audit readiness and response initiatives, including NC Medicaid, NCQA, and EQR reviews, ensuring full compliance with Tailored Care Management documentation, service delivery, and performance standards.
• Ensure that all care management activities are conducted in a person-centered, trauma-informed, culturally competent manner, compliant with NC Medicaid Tailored Plan requirements, while focusing on enhancing health outcomes, promoting health equity, and facilitating successful community reentry.
• Associate’s degree coupled with four (4) years of experience in behavioral/mental health, substance use, and/or intellectual/developmental disability populations; OR a Bachelor’s degree with two (2) years of relevant experience; OR an equivalent combination of education and experience.
• Possession of a valid driver’s license is required.
• Candidates must reside within 75 miles of Trillium’s Catchment area in North Carolina.
• A minimum of one (1) year of experience in criminal justice or juvenile justice systems is preferred.
• At least two (2) years of supervisory experience managing a team of two or more employees is preferred.
• Education, training, and work experience that demonstrate a history of acquiring and applying the necessary skills to engage with a diverse array of individuals from various socio-economic, cultural, ethnic, educational, and other backgrounds.
