Senior Integrated Clinic Pharmacist

This is a fully remote position, open to applicants in South Carolina.

📋 Description

• Utilize evidence-based medicine and clinical expertise to assess the appropriateness of pharmacotherapy for patients with chronic conditions, such as diabetes, hypertension, heart failure, and COPD.

• Implement clinical protocols as permitted by the organization and state regulatory bodies.

• Enhance drug coverage benefits to ensure optimal care and minimize costs (including preferred formulary products, lower-cost alternatives, and home delivery pharmacy services).

• Collaborate with physicians, nurses, and other healthcare professionals to elevate patient care quality.

• Engage in high-risk rounds with a multi-disciplinary team to evaluate patient progress and modify treatment plans accordingly.

• Possess strong patient counseling abilities to convey complex medical information effectively to patients and healthcare providers.

• Take part in quality improvement initiatives focused on enhancing medication safety and efficacy.

• Conduct post-discharge medication reconciliation for patients at high risk during transitions of care.

• Record all activities within the clinic's EMR and any other necessary pharmacy or clinical management systems.

• Support CMS Stars and HEDIS quality measures to ensure successful clinic outcomes.

• Maintain thorough documentation of clinical interventions and patient care activities.

• Lead in-service education sessions for healthcare staff and create educational materials pertaining to pharmacotherapy.

• Stay updated on advancements in pharmacotherapy and current clinical guidelines.

• Provide consultations to healthcare providers regarding complex medication management challenges.

• Address additional requests from the Primary Care Organization, offering support within the pharmacist's defined scope of practice to further organizational objectives.

• Execute targeted initiatives or priorities as instructed by the National Director or Associate Directors of Pharmacy, adhering to best practices and regulatory standards.


⛳️ Requirements

• Active and unrestricted Pharmacist license.

• Willingness to obtain licensure in additional states as required for market support.

• Pharm.D. or another advanced pharmacy degree.

• A minimum of 2 years of experience as a practicing pharmacist.

• Intermediate or higher proficiency in Microsoft Office applications such as Word, Excel, and PowerPoint.
