Senior Integrated Clinic Pharmacist

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Apply evidence-based medicine and clinical expertise to assess the appropriateness of pharmacotherapy for patients at high risk with chronic illnesses, such as diabetes, hypertension, heart failure, and COPD.

• Implement clinical protocols as permitted by the organization and state regulations.

• Enhance drug coverage benefits to ensure optimal care while minimizing costs (including preferred formulary products, cost-effective alternatives, and home delivery pharmacy services).

• Collaborate with physicians, nurses, and other healthcare professionals to elevate patient care standards.

• Engage in high-risk rounds with a multi-disciplinary team to evaluate patient progress and modify treatment plans as necessary.

• Exhibit outstanding patient counseling abilities to relay intricate medical information to both patients and healthcare providers.

• Take part in quality improvement initiatives focused on bolstering medication safety and efficacy.

• Conduct post-discharge medication reconciliation for patients experiencing high-risk transitions of care.

• Record all activities in the clinic's EMR and other essential pharmacy or clinical management systems.

• Support CMS Stars and HEDIS quality metrics to ensure successful clinic performance.

• Maintain precise documentation of clinical interventions and patient care activities.

• Facilitate in-service educational sessions for healthcare staff and create and disseminate educational resources related to pharmacotherapy.

• Remain updated on the latest advancements in pharmacotherapy and clinical guidelines.

⛳️ Requirements

• Possession of an active and unrestricted Pharmacist license.

• Willingness to obtain licensure in additional states as required for market support.

• Pharm.D. or another advanced pharmacy degree.

• A minimum of 2 years' experience as a practicing pharmacist.

• Intermediate proficiency or higher in Microsoft Office applications, including Word, Excel, and PowerPoint.
