Pharmacist – Hemophilia

This is a fully remote position, open to applicants in California, +4 more states.

📋 Description

• Ensure that patients receive the appropriate medication, comprehend its usage, and address any drug therapy issues before dispensing.

• Review and accurately input prescription orders.

• Offer patient counseling and education regarding medications and treatment plans.

• Confirm the completeness and accuracy of prescription data entry.

• Conduct drug utilization reviews and address therapy-related concerns.

• Collaborate with clinics to clarify and secure prescriptions.

• Coordinate and engage in pharmacy services, dispensing, quality assurance, and clinical monitoring.

• Evaluate pharmaceutical needs and plan service delivery.

• Take part in patient care plans as well as quality assessment and improvement initiatives.

• Assess the appropriateness of prescriptions, including dosage, drug choice, interactions, route, and stability.

• Oversee activities related to filling, receiving, compounding, and dispensing.

• Maintain comprehensive patient charts and ensure that necessary laboratory tests are ordered, obtained, and discussed.

• Provide patient counseling, education, monitoring, and clinical interventions.

• Assess therapy response, adherence, and drug regimens; report and manage any adverse events.

• Assist physicians, nurses, and patients with their clinical and technical needs.

• Document all communications in patient charts.

• Ensure compliance with paperwork required by state and federal agencies as well as the Joint Commission.

• Support the Pharmacy Manager in procedures, training, and supervision of pharmacy personnel.

• Engage in performance improvement projects and in-service programs.

• Fulfill education, training, competency, and skills assessment requirements.

• Partake in the on-call schedule.

• Perform additional tasks as assigned by the Pharmacist-In-Charge and adhere to policies and standards.

⛳️ Requirements

• Must reside in Florida, Texas, Missouri, California, or Michigan.

• Must possess and maintain an active pharmacist license in one of these states.

• Bachelor’s degree in pharmacy and 3+ years of experience in mail order, retail, or hospital pharmacy OR Pharm D.

• Experience in hemophilia pharmacy is highly preferred.

• Strong verbal and written communication abilities.

• Excellent problem-solving and critical-thinking skills.

• Outstanding organizational and time management skills.

• Collaborative and adaptable mindset in a fast-paced environment.

• Patient-focused dedication to high-quality specialty care.

• Availability to work the relevant Eastern, Central, or Pacific schedule from Monday to Friday.

• Must complete competency and skills assessment requirements.
