This is a fully remote position, open to applicants in Illinois.
• Lead strategic analyses, generate data-driven insights, and manage reporting.
• Examine data to guide strategic decision-making concerning patient retention.
• Create and extract reports on performance metrics related to patient retention.
• Conduct analyses to pinpoint areas of progress and opportunity.
• Share insights with local and organizational leaders.
• Enhance patient retention at Oak Street by executing Patient Engagement and Retention workflows.
• Cultivate relationships with key partners across provider services, Salesforce, and patient communications and marketing.
• Oversee proactive workflows associated with patient communications and panel reassignment.
• Support key organizational retention goals by implementing initiatives aimed at achieving annual patient wellness visit completion targets.
• Recognize essential factors influencing annual wellness visit completion and target patient populations for outreach.
• Analyze the effectiveness of various engagement strategies in reaching annual wellness visit completion objectives.
• Serve as project manager for ongoing initiatives and activities related to patient segmentation.
• Develop metrics and reports to track success and uncover opportunities for improvement across new initiative implementations.
• Perform other duties as assigned.
• A minimum of 2-4 years of relevant experience.
• Strong organizational skills and attention to detail.
• Proven experience collaborating on projects across various departments.
• Proficient in Microsoft Excel or other standard analysis tools (SQL, Alteryx, etc.).
• Demonstrated intellectual curiosity and problem-solving capabilities.
• Ability to navigate through leadership and operational complexities.
• Commitment to excellence, willing to go above and beyond to achieve objectives and ensure consistently high-quality work.
• Quick learner who is comfortable with ambiguity.
• Strong sense of accountability and discipline regarding timelines and deliverables.
• Collaborative working style with the ability to work cross-functionally.
• Bachelor’s degree is required.
