This is a fully remote position, open to applicants in United States.
• Arrange appointments with external network specialists for consultations and procedures requested by PACE Primary Care Physicians.
• Prioritize the scheduling of appointments based on Expected By Date and clinical urgency.
• Collaborate on a daily basis with clinical teams to manage participant flow and ensure timely access to care.
• Meet or surpass scheduling productivity standards.
• Accurately record appointment data and updates in Epic.
• Serve as a liaison between participants, caregivers, providers, and external specialist offices to coordinate care and resolve scheduling conflicts.
• Inform participants and families about appointment details.
• Schedule complex medical appointments and manage referrals and specialty care services.
• Obtain and monitor consultation notes and diagnostic testing results.
• Manage clinic-based scheduling workflows and participant care coordination activities.
• Provide front-office clinic support, including participant check-in and appointment management.
• Prepare and coordinate authorizations for specialty referrals and services.
• Document scheduling activities and participant interactions within the EMR.
• Work collaboratively with the Interdisciplinary Team, Transportation Department, Adult Day Health Center, and clinic staff.
• Participate in IDT meetings as necessary.
• Coordinate schedule modifications, transportation adjustments, medically necessary escorts, and coverage requirements.
• Support internal audits, quality reviews, and process improvement initiatives.
• Execute other duties as assigned by the Regional Scheduling Manager.
• A minimum of 1 year of administrative, clerical, and medical administrative experience, including filing, record keeping, data entry, and customer service.
• At least 1 year of scheduling experience.
• Recent experience in medical scheduling within the past 1–2 years of employment.
• 3–5 years of customer service experience.
• Experience working remotely.
• Familiarity with medical terminology.
• Experience with EMR systems.
• Capability to perform essential duties satisfactorily.
• Preferred: clinic healthcare experience in a geriatrics setting.
• Preferred: experience as a Nursing Assistant in home care, long-term care, daycare, or group home settings.
• Preferred: experience using Epic EMR.
• Florida applicants must complete background screening through the Florida Care Provider Background Screening Clearinghouse.
