This is a fully remote position, open to applicants in Alabama, +14 more states.
β’ Conduct virtual primary care consultations through telehealth, encompassing comprehensive evaluations, management of chronic diseases, preventive screenings, and acute care interventions.
β’ Assess and manage chronic conditions (such as hypertension, diabetes, COPD, and heart disease) while creating tailored care plans to enhance patient health.
β’ Order and analyze lab tests, imaging, and other diagnostic procedures, coordinating necessary follow-up care.
β’ Oversee medication prescriptions, including controlled substances, titrations, and refills, ensuring patient safety and adherence.
β’ Deliver health coaching, guidance on lifestyle modifications, and patient education to empower individuals in managing their health effectively.
β’ Identify patients who require referrals or home-based services, and facilitate appropriate coordination with specialists or community resources.
β’ Utilize and provide feedback on our electronic health platform, contributing to its ongoing enhancement to improve both patient and provider experiences.
β’ Engage in clinical case reviews, team meetings, and interdisciplinary collaborations to ensure the delivery of high-quality, patient-centered care.
β’ Commit to continuous learning and improvement, remaining current with best practices in both virtual and primary care management.
β’ Possess an active Advanced Practice Provider (APRN or PA) license in good standing across AL, AZ, AR, CO, FL, GA, HI, IL, IN, KY, MS, MO, NC, OH, PA, SC, TN, TX (must be willing to obtain additional licenses and a DEA certification within the first 90 days β support and reimbursement provided).
β’ Hold board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
β’ Have a minimum of 2 years of experience in primary care, urgent care, or internal medicine, preferably with a background in telehealth or virtual care settings.
β’ Demonstrate experience in managing chronic conditions, preventive care, and acute illnesses through telehealth.
β’ Familiarity with value-based care models, population health management, and quality metrics (HEDIS, STAR ratings, etc.) is advantageous.
β’ Comfortable navigating digital documentation and telemedicine platforms (experience with EHRs and virtual care workflows is preferred).
β’ Maintain active BLS certification at the time of hire.
β’ Have a minimum of 6 months of experience as a Nurse Practitioner in the D2Me program.
β’ Be flexible with shift times to align with standard business hours (9:00 AM β 5:00 PM) in the patient's local time zone.
