ROOTS Hub Director

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Act as Vaya’s liaison for the RHTP with the Department and other awarded ROOTS Hub Lead entities regarding program-related roles and responsibilities.

• Supervise and ensure effective implementation and adherence to the NC DHHS scope of work and Vaya’s RHTP plan.

• Collaborate within the organization and with lead anchor partners to guarantee all RHTP responsibilities and requirements are fulfilled.

• Manage the operational processes of the Hub’s intake, screening, merit review, technical advisory review, regional recommendation, and final funding approval.

• Keep the project plan updated and provide consistent progress reports to the supervisor and Vaya leadership on RHTP accomplishments and challenges.

• Implement the Hub Action Plan in the region defined by NC DHHS.

⛳️ Requirements

• A Bachelor’s degree is required.

• At least two (2) years of experience in project management for large and complex projects.

• A minimum of two (2) years of experience coordinating across various stakeholder types.

• A minimum of two (2) years of experience in a role or organization that shows an understanding of healthcare systems, Medicaid and/or Medicare payer systems, and the clinical and social needs affecting communities’ and/or populations’ access to health services.

• Must be a resident of North Carolina and be hired by June 1, 2026 [proposed date].

• Proven success in stakeholder relations.

• Previous project management experience is required.
