This is a fully remote position, open to applicants in Ohio.
• Oversee and finalize the Hospice Election Addendums (HEAs) process.
• Assess missed Hospice Outcomes and Patient Evaluation (HOPE) assessments as well as Symptom Follow-Up Visit (SFV) assessments.
• Ensure that patients and families receive necessary HEA information within mandated timeframes.
• Conduct audits of HOPE visit requirements and documentation to uphold hospice quality and regulatory adherence.
• Work collaboratively with interdisciplinary team members and admission staff to gather, document, and communicate precise patient-specific information.
• Foster an outstanding patient and family experience throughout the HOPE assessment process.
• Identify and offer educational and collaborative opportunities for the Care Team, either individually or in group settings.
• Supervise Repair & Maintenance, Culinary and Hospitality, as well as Ohio Department of Health requirements for facilities.
• Collaborate with colleagues while working remotely.
• Full-time position from 8:30 AM to 5:00 PM.
• Registered Nurse (BSN preferred) licensed in the state of Ohio with no board actions.
• At least 2 years of professional nursing experience; hospice experience is preferred.
• Background in hospice, quality, regulatory compliance, clinical auditing, or a related healthcare quality function is preferred.
• Proven knowledge of hospice regulatory standards, clinical documentation guidelines, and quality processes.
• Experience in reviewing clinical documentation and identifying missed, incomplete, or non-compliant assessments and follow-up needs.
• Excellent communication, organization, critical-thinking, and collaboration skills, with the ability to work independently and manage multiple priorities.
• Experience in effectively working in a remote/virtual environment and collaborating with interdisciplinary team members is preferred.
• Valid driver’s license with a safe driving record.
