This is a fully remote position, open to applicants in California.
• Advocate for and educate new patients from the initial intake phone assessment to their first visit with the onsite care team.
• Guide new patients through the intake process utilizing assessment, planning, and evaluation methods.
• Determine the clinical suitability of patients for safe travel to City of Hope.
• Act as a liaison among patients, families, multidisciplinary teams, and providers to coordinate first appointments and streamline initial evaluations.
• Evaluate cancer treatment history, including the diagnosis, treatments received, physicians involved, and facilities used.
• Collaborate with OIS representatives and new patient schedulers to assess the appropriateness of visits to the organization.
• Conduct patient interviews to determine clinical suitability for care at affiliated facilities and clinical trials.
• Complete preadmission clinical evaluations and recommend care options based on diagnosis, providers, treatment possibilities, and financial considerations.
• Ensure prompt documentation in the EMR and obtain current outside medical records and imaging.
• Support patients throughout treatment decisions, identify and resolve barriers, and prevent gaps in care.
• Facilitate or assist with admissions during the intake process.
• Collaborate with intake, medical oncology, specialty clinics, and CAM teams to create care plans and educate patients and their families.
• Facilitate multidisciplinary patient education while addressing concerns of patients and families.
• Serve as a liaison between Oncology Information Services and Medical Center care providers for service recovery purposes.
• Support departmental goals and participate in performance improvement initiatives.
• Attend mandatory staff meetings and plan workflows to ensure timely completion of responsibilities.
• Ensure that encounters and new charts are complete with necessary physician documentation.
• Assess special travel needs and coordinate details with support staff.
• Employ problem-solving techniques in the intake evaluation process.
• Demonstrate understanding of patient safety goals, variance reporting, disaster codes, hand hygiene, and transmission-based precautions.
• Comply with facility policies, procedures, and Joint Commission standards.
• Graduate from an accredited nursing school.
• Possession of a state-issued Registered Nursing (RN) license.
• Successful completion and verification of the California state registered nursing license within 90 days of employment.
• Basic Life Support (BLS) certification.
• Oncology Certified Nurse certification is highly preferred.
• Availability to work Monday–Friday, with 8-hour shifts scheduled between 7 a.m. and 7 p.m. Pacific Standard Time.
• Understanding of patient safety goals relevant to the work environment.
• Familiarity with hospital variance reporting systems and appropriate management procedures.
• Knowledge of organizational disaster codes.
• Completion of annual Hazardous Communications Training.
• Fulfillment of required CE Direct continuing education.
• Adherence to organizational hand hygiene and standard/transmission-based precaution guidelines.
