This is a fully remote position, open to applicants in Illinois.
• Collaborates with the pod leader to coordinate care services, ensuring that patients have access to a complete range of services tailored to their individual needs throughout their healthcare experience.
• Works in partnership with the care team to create and manage personalized care plans, address gaps in care, and connect with additional resources to guarantee access to necessary services.
• Oversees the transition of care for patients throughout their healthcare journey to ensure that patient needs are met appropriately, preventing avoidable hospital admissions.
• Facilitates High Risk Huddles and ensures the completion of follow-up actions.
• Prioritizes patients based on the urgency and severity of their conditions to ensure that the most critical cases receive prompt attention.
• Reviews medical records to identify care gaps and coordinates with the care team to manage these issues effectively.
• Regularly revises patient care plans.
• Conducts thorough nursing assessments via telephone to enhance or optimize current health outcomes.
• Educates patients and/or their caregivers about disease management, medication, health maintenance, and disease prevention to encourage self-management and improve health outcomes.
• Demonstrates strong clinical skills, critical thinking, and effective communication in interactions with patients, caregivers, providers, and fellow care team members.
• Accurately documents necessary interactions, assessments, and updates in patients’ medical records according to established processes and guidelines.
• Acts as a liaison between patients, providers, and resources to ensure seamless care delivery.
• Facilitates communication regarding patient status and care plans during transitional experiences, such as from home to hospital, hospital to post-acute care, and back to home.
• Current Registered Nurse License.
• A minimum of 2 years of experience in care management within community, health plan, or hospital systems.
• Strong clinical skills and proactive thinking abilities.
• Excellent communication skills.
• Capability to perform extensive assessments over the phone.
• Knowledge of Medicare regulations as well as home care and hospice standards.
• Experience with small group presentations and teaching/training.
• Exceptional interpersonal skills.
• Outstanding written and verbal communication skills.
• Proficiency in computer programs (email, Word, Excel, PowerPoint, etc.).
• Efficient time management skills to ensure all duties and documentation are completed punctually.
