This is a fully remote position, open to applicants in California.

📋 Description

• Act as the primary support for seniors as they navigate various social and clinical scenarios.

• Address health-related social factors, arrange transportation, and facilitate access to food assistance.

• Assist in managing chronic illnesses and enhance health literacy among patients.

• Foster enduring relationships with both patients and healthcare providers.

• Guide patients through the complexities of the healthcare system and clarify insurance documentation.

• Empower patients to take charge of their health management.

⛳️ Requirements

• Valid California Registered Nurse (RN) license in good standing.

• Must reside within the United States.

• Profound knowledge of social determinants of health (SDOH).

• Experience in working with diverse and vulnerable patient demographics.

• Outstanding active listening and interpersonal communication skills.

• Strong organizational and administrative capabilities.

• Willingness to learn a new electronic health record (EHR) system and embrace AI tools.
