RN Health Advocate – 1099

This is a fully remote position, open to applicants in United States.

📋 Description

• Act as the primary support for seniors as they navigate both social and clinical challenges.

• Eliminate obstacles to assist patients in achieving optimal health outcomes.

• Address social determinants of health (SDOH) while coordinating a variety of assistance resources.

• Oversee and support patients managing chronic conditions by enhancing health literacy.

• Establish long-lasting, trustworthy relationships with patients to help them meet their healthcare objectives.

• Work in partnership with physicians to create customized care plans.

⛳️ Requirements

• Current Compact or California Registered Nurse (RN) license in good standing.

• Thorough understanding of social determinants of health (SDOH) and experience serving diverse, vulnerable patient populations.

• Outstanding active listening and interpersonal skills.

• Strong organizational abilities and effective communication skills.

• Proficient in learning new electronic health records (EHR), utilizing AI tools, and adapting to changing operational processes swiftly.

• Demonstrate resilience, persistence, and creativity in overcoming barriers within the US healthcare system.

• Preferred: Experience in community health, health coaching, or working with Medicare populations.

• Preferred: Proficiency in Spanish or additional languages.
