This is a fully remote position, open to applicants in United States.
• Stabilize high-risk IKC patients by providing intensive care coordination aimed at reducing hospital readmissions.
• Assist the high care pod team in delivering exceptional services.
• Organize and facilitate quality, cost-effective care while reducing fragmentation for ESRD and CKD patients.
• Coordinate care among patients, families, physicians, providers, care teams, the community, and the Regional Operations Manager.
• Oversee clinical, financial, and patient satisfaction outcomes.
• Implement change-agent practices and Continuous Quality Improvement (CQI) techniques.
• Offer healthcare performance coaching to enhance team effectiveness.
• Adjust practice patterns to align with new electronic processes and productivity expectations associated with Capella implementation.
• Utilize DaVita clinical software, including Capella, in daily operations.
• Collaborate independently and with the healthcare team to ensure high-quality patient care.
• An Associate Degree in Nursing (A.D.N.) from an accredited institution is required; a three-year diploma from an accredited program may be accepted as a substitute.
• A Bachelor of Science in Nursing (B.S.N.) is preferred.
• A current Registered Nurse (RN) license in the state of practice is mandatory.
• Continuing education credits must be maintained as required by the state of practice.
• A minimum of five (5) years of clinical nursing experience is required.
• At least three (3) years of renal nursing experience is preferred.
• Proven knowledge and understanding of data management relating to clinical, financial, and patient satisfaction outcomes.
• Demonstrated effectiveness in a change-agent role is essential.
• Familiarity with CQI techniques is necessary.
• Previous experience in healthcare performance coaching is required.
• Certification as a Nephrology Nurse (CNN) or Certified Case Manager (CCM) is preferred.
• A current CPR certification is essential.
• Ability to adapt to new electronic processes and elevated productivity expectations related to Capella implementation.
• A valid driver's license in the state of employment, a positive driving record, and the ability to meet insurance coverage criteria are required.
• Basic computer skills, along with proficiency in MS Word and Outlook, are required.
• Functional proficiency with DaVita-specific clinical software programs, including Capella, must be achieved within 90 days of employment.
• A home office setup with internet connectivity of at least 1MB upload and 1MB download speed is required.
