RN Case Manager – Care Transitions, Bilingual Spanish

This is a fully remote position, open to applicants in California.

📋 Description

• Oversee and coordinate healthcare services for members who have complex and chronic care requirements within the bounds of licensure.

• Evaluate, develop, implement, coordinate, monitor, and assess care plans throughout the care continuum.

• Supervise and track member progress and services to guarantee consistent, cost-effective care that adheres to policies and regulations.

• Develop personalized discharge plans in collaboration with patients, families, and the care team.

• Facilitate care coordination with providers, rehabilitation facilities, and home health agencies.

• Track patient progress and modify care plans as necessary.

• Educate and encourage patients and families in self-care and follow-up practices.

• Advocate for patient requirements and connect them with community support services.

• Engage in care conferences, quality improvement efforts, and interdisciplinary collaboration.

• Primarily perform duties via telephone.

⛳️ Requirements

• A minimum of 5 years of clinical case management experience, or a relevant combination of education and experience.

• Active and unrestricted RN license in California.

• Willingness to acquire additional state licenses.

• Understanding of Medicare Managed Care Plans, insurance regulations, and community resources.

• Excellent communication, problem-solving, and organizational abilities.

• Capability to perform essential physical functions, including standing, walking, sitting, using hands, reaching, and frequently lifting or moving items weighing up to 10 pounds.

• Bilingual proficiency in Spanish is required as specified in the job title.
