This is a fully remote position, open to applicants in New York.

📋 Description

• Perform comprehensive case management assessments and create care plans.

• Work in conjunction with the care team to assess patient needs and coordinate appropriate care.

• Record all patient interactions and activities related to care coordination.

• Manage patient calls and electronic communications, escalating issues when necessary.

• Assist prescribers with prior authorizations, medication refills, pharmacy coordination, and scheduling based on clinical requirements.

• Carry out medical reviews and collect additional patient history as required.

• Secure and manage necessary patient consents and releases of information.

• Educate and onboard patients into AptiHealth’s Medication Management Program.

• Evaluate patient responses to treatments and medications.

• Link patients to community resources and support services to address social determinants of health.

• Conduct proactive telephonic outreach to patients in alignment with clinical needs.


⛳️ Requirements

• Current Registered Nurse (RN) license in New York is mandatory.

• Must be a graduate of an accredited nursing program; a BSN is preferred.

• Certification as a Certified Case Manager (CCM) is preferred.

• Strong preference for candidates with experience in behavioral health or psychiatric care.

• Familiarity with care coordination and case management workflows is essential.

• Proficient in using Electronic Medical Record (EMR) systems and Microsoft Office applications.

• Excellent organizational, communication, and clinical triage abilities.

• Must be capable of working collaboratively within a multidisciplinary care team.

• Comfortable in a fast-paced, patient-centered virtual care setting.
