RN Care Manager

📋 Description

• Overseeing the entire care continuum for assigned patients from admission to discharge.

• Demonstrating a thorough understanding of the RN scope of practice and current state regulations.

• Collaborating with the multi-disciplinary team and applying scientific evidence to ensure best practices.

• Facilitating the discharge planning process in partnership with the interdisciplinary team.

• Recording all communications and concerns related to the coordination of care and services.

• Engaging with physicians about the level of care or admission status when criteria are not fulfilled.

• Recognizing patients and families with psychosocial and/or non-medical needs during discharge planning.

• Contributing to Performance Improvement and the Evidence-Based Practice/Research initiatives.

⛳️ Requirements

• Bachelor's Degree in Nursing, Associate Degree in Nursing (ADN), or Diploma in RN.

• A minimum of three (3) years of RN experience along with current BLS certification.

• Active Registered Nursing License in Arkansas.

• National Nursing Certification preferred; BSN preferred; Certified Case Manager (CCM) or American Case Management (ACM) certification is a plus.
