This is a fully remote position, open to applicants in Virginia, +1 more state.
• Conduct home visits with participants at least every six (6) months, adhering to established timelines.
• Exhibit a thorough understanding of the Personal Options services program, relevant West Virginia waiver programs, and Palco policies.
• Provide education and mentorship to participants and/or their designated representatives regarding their rights, responsibilities, and potential risks associated with the program.
• Assess and evaluate the capability of participants or their representatives to engage effectively in the program.
• Support members and their representatives, as needed, in creating and updating their initial service/spending plan.
• Oversee the utilization of service/spending plans.
• Address and respond to all complaints and grievances from members/representatives concerning FMS and Resource Consulting services.
• Act as a mandatory reporter, responding to and reporting all necessary incidents, including allegations of suspected abuse, neglect, and exploitation.
• Employ a person-centered approach in all participant interactions.
• Accurately document all participant visits using a company-issued laptop with professional and detailed language.
• Log events and generate reports electronically.
• Carry out essential administrative tasks daily, such as scheduling and confirming upcoming visits and responding to company communications.
• Adhere to operational procedures outlined by Palco.
• Participate in shadowing sessions with new resource consultants as requested and attend periodic training sessions mandated by Palco.
• Timely distribute new information to participants within your caseload.
• Handle company equipment and data with care, following company policies, including HIPAA privacy practices.
• A minimum of three years of experience in public-facing roles is essential.
• Preferred experience includes roles such as patient navigator, community connector, community health worker, lay health advisor, health educator, or case manager.
• A High School Diploma or equivalent is required.
• Professional verbal and written communication skills are necessary.
• Ability to document visits electronically is required.
• Proficiency in Microsoft Office, Microsoft Outlook, and Internet use is essential.
• Strong problem-solving abilities are necessary.
• Willingness to travel within a designated geographic area to meet participants at their homes is required.
• Ability to work with individuals of all ages and disabilities is essential.
• A valid driver’s license and reliable transportation are necessary.
• Must have and maintain an active prepaid or contract cell phone plan for ongoing communication with participants, management, and/or company leadership.
• Must successfully pass a criminal background check and a maltreatment check.
