Registered Nurse – Virtual Care, Care Management

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee the care of medically complex patients by developing and monitoring care plans across various programs (CCM, RPM, RTM, APCM).

• Complete preparation for Annual Wellness Visits (AWV) charts.

• Proactively address care gaps, coordinate orders, and promote preventive and chronic care initiatives.

• Prepare detailed AWV charts to enhance provider efficiency and ensure compliance with CMS.

• Conduct face-to-face virtual visits via Zoom to review and summarize findings with patients.

• Provide concise patient summaries and highlight key findings to providers ahead of each AWV.

• Deliver telephonic and portal-based care management for patients with complex medical needs.

• Perform comprehensive nursing assessments that encompass physical, functional, psychological, social, environmental, and learning aspects.

• Create, execute, and oversee personalized care plans for CCM, RPM, RTM, and APCM.

• Track patient progress, reassess objectives, identify obstacles, and modify care plans as necessary.

• Offer patient education, coaching, and support tailored to health literacy and cultural considerations.

• Serve as a patient advocate and assist in problem-solving efforts.

⛳️ Requirements

• Valid and unrestricted Registered Nurse (RN) license in a compact state.

• Willingness and ability to obtain additional state licenses upon hiring (company-funded).

• A minimum of 2–3 years of clinical nursing experience.

• Experience in providing care in a telehealth or virtual care environment.

• Familiarity with remote work.

• Experience in one or more of the following areas:

• Chronic Care Management (CCM)

• Remote Patient Monitoring (RPM)

• Telehealth or virtual care coordination

• Primary care or care management

• Proficient in EHR systems and telehealth platforms.

• Strong clinical assessment and care coordination abilities.

• Exceptional verbal and written communication skills.

• High degree of organization, attention to detail, and effective time management.

• Capability to work independently in a remote setting.

• Commitment to providing patient-centered and culturally sensitive care.

• A dedicated, private workspace at home.
